How to fix Umbraco Contour error after upgrade from Umbraco 4.0.3 to 4.0.4.2

Today I realized that upgrading from Umbraco 4.0.3 to Umbraco 4.0.4.2 caused the Umbraco Contour to not able to create a new Form. It throws an error every time I tried to create a new Form.

Here is the fix of it.

Open /umbraco/config/create/UI.xml and add the following codes:
